Insurance Information

A number of different insurance options are available at Longevity Medical Health Center. We are able to bill most commercial insurance companies* for many of the services provided at our clinic. In order to determine what services your insurance plan may provide coverage for please provide us with the following information:
  1. Your full name
  2. Date of Birth
  3. Your phone number and email address
  4. The name of your insurance company
  5. The type of insurance plan you have (HMO, POS , PPO, etc.)
  6. Your insurance subscriber number or member number
  7. The member services telephone number for your insurance company
Please contact our office at 602-493-2273 to provide this information. Alternately, this information can be sent via email or by fax at 602-493-2159. If faxing, please include a photocopy the front and back of your insurance card along with your date of birth.

*Please note that Medicare does not cover services provided by naturopathic physicians, therefore, we are not able to bill Medicare claims.
